After filling out the form and clicking on the "Submit" button, you will be redirected to a confirmation page.
You will receive a follow-up confirmation of your order by email or phone.
Receipt of payment through the mail is required before purchases will be filled.
Make check payable to the Kentucky State Treasurer.
Kentucky residents must add 6% sales tax to their Subtotal.
