Staff will conduct a search for ONE PERSON in ONE RECORD.

Submit only one form at a time and wait for a reply before submitting another request.

In-state Customer Fees

In-state customers must submit a $10.00 non-refundable search and copy fee with each genealogical reference request. A check or money order should be payable to the Kentucky State Treasurer. Do not send cash.

Out-of-state Customer Fees

Out-of-state customers must submit a $15.00 non-refundable research fee with each genealogical reference request. Staff will not undertake any research unless this fee is pre-paid. Check or money order should be payable to the Kentucky State Treasurer. Do not send cash.

Please note: Additional postage and copy charges (50¢ per microfilm copy, and 25¢ per photocopy) will be applied to requests that require the reproduction of extensive records consisting of more than six microfilm copies and/or 12 photocopies.
